Remove Application Fee from Cost.php #70

Please remove the link and description to the Application fee from the Cost.php page. We only want that link at the bottom of the application page.

Hi There... this is now complete. FYI - You can add or remove ticket items from cost.php by checking or unchecking the "Active" status of the ticket item. This is found under the ticketing menu.
