Use Boostrap4 custom elements in forms #63

Low priority, but it would be cool to use nice Boostrap custom-* form elements instead of the default (and often ugly) browser defaults.

I first thought django-crispy-forms 1.7.1 would support this out of the box, but apparently not. As far as I can tell, I'd need to modify the classes manually somehow. Worth investigating at one point, but not right now..
