Document the extensions the starter pack includes #125

Add a section to the readme that explains which extensions the starter pack uses by default, and link to their documentation.

Also make it as clear as possible that users are supposed to edit the custom_conf.py file only and which configuration in there is required and which is for advanced usage/custom configuration.
