Describe the bug
On EventForm, if desiring to select a standard form and an additional 'other' form, selecting 'other' deselects display of standard form selections. They are still selected because upon event save, the "deselected" values still appear in the list of selected items, but the display shows deselected.

Steps to reproduce the behavior:
Add/Edit event
Select standard form
Select other form
See standard form deselect
Expected behavior
User should be able to select a mix of standard and other form types
