Add comment “generated with ceur-make on YYYY-MM-DD” to output #11

Fixed in a different way: including the new CEURVERSION annotation, which refers to the version of Vol-XXX/index.html. That's really what matters, that we use the latest version of the template.

No, timestamp of running ceur-make is helpful, as it allows us to identify ToCs that have been created with ceur-make (and not copied from old ceur-make-generated ones).
