Add a transformer for the CSQ effect annotation field (similar to ANN). E.g., used in ExAC VCFs.

Hi @pedrostrusso, sorry I'm swamped at the moment, unlikely to get to this in the short term. Happy to give guidance if anyone wanted to make a PR.
