Feature CPD-884 & 887 & 888: Remove template links and opened stats, add click rates for indicators

Templates in report card do not have active links. Removed references to 'opened' data. Show the click rate percentage for each indicator in the 'Most Clicked Indicators' section of the report card.

💭 Motivation and context

Requested by the client. We consider 'opened' to be an unreliable metric. The links in the report are unnecessary, and we don't want administrators clicking around the report to contribute to stats.
