Need to Add "A signed copy of the "Terms and Conditions of License" must be submitted with the insurance documents." to the Applications item #11.

It currently reads "11. Proof of insurance documentation that names the City of Austin as an additional insured"

Item 11. on the Application would read "Proof of insurance documentation that names the City of Austin as additional insured, and a signed copy of the "Terms and Conditions of License" must be submitted."

Describe any workarounds you currently have in place or alternative solutions you've considered.

we add a comment that says they need to upload both documents

• Was syncing with Andrea about something on slack, I remembered I had to tell her about this.

  Oh, I forgot to mention to you we had completed that update to the Terms and Conditions earlier in November..I failed to tell you on 11/15, my bad.
