Add Conditions field to the permit report templates for RW-Valet Annual and RW-Valet Temporary.

Acceptance Criteria

The RW-Valet Annual and RW-Valet Temp permit documents that are generated from Folder Tab need to be updated.
A new line needs to be added after the row showing the Dates.
The new line should say 'Conditions:' and pull in the Folder Condition data into the permit document.

ROW will be able to include permit conditions specific to valet zone permits that are visible on the permit itself.

Leaving the conditions in AMANDA or emailing them to the applicant.

We need to meet with ROW to redesign the permit forms. Where do we put the conditions?
