I was unable to identify with certainty what caused the issue. There were four other items created on that day that did not result in duplicate records. I did notice that we did not have any "Must be unique" rules set for the stock number field in the Data Tracker. I activated that and it should prevent further issues like this.

Regarding duplicate items (aka with the same commodity code): I went through our June 28 logs and it appears that the integration script failed due to an internet connection issue while importing this new item (550-85-590023) from the Finance system. The failure caused the script to run again despite having already created this item once.
I added additional safeguards in the Data Tracker to prevent this from happening again. The system will throw an error if an item's stock number already exists in the system. These safeguards are already in place in the Finance system, but I must have forgotten to activate them in the Data Tracker when I completed the integration.
To be safe, I also ran a search for additional duplicate items and did not find any.
Ronnie identified a duplicate item in the Data Tracker (550-85-590023). It appears that the integration created the item twice when Johnny P created it on 6/28.
