Remove PP stubs and reformat predefine_encodings() #104

Encode does not work without XS for a long time, so we do not need PP stubs in code anymore. After removing it allow us to cleanup and reformat boot code in function predefine_encodings().

Travis failed for perl 5.16 and 5.20 but looks like Travis is temporary unavailable (empty log). Let me pull and see if it reproduces...
