Fix 0x95 #41

Both http://www.unicode.org/Public/MAPPINGS/VENDORS/MISC/KOI8-U.TXT and RFC2319 provide "BULLET OPERATOR" instead of "BULLET" for 0x95.

Also, fix URL in header comment.

I wasn't able to trace the source of this koi8-u.ucm. The original enc file was probably imported from TCL, but at least since 2001 the TCL instance correctly specifies U2219 instead of U2022.
