This PR is based on discussion from the restrospective #32 and should address discussion in #24

It:

[x] adds contributing guidelines that point to the main DT coc and contributing (fixes #25)
[x] add license info on notes
[x] updates facilitation section of readme
[x] shuffles/streamlines the readme for 2019 reading group
