8: Add club models and migration #34

This adds the migration and models for club functionality. Associations are not required, viewable, or selectable on the UI yet.

LGTM. I think something to note (and maybe we'll want to capture this in another issue) is that eventually we'll want to add locations to clubs. Those aren't in place now (I'm working on those at the moment) but once they are we'll want to update this.
