Display feed icons in the main navigation only if requested.
Categories can now be display in the main navigation independently of the setting if pages are displayed or not.
a new MENUITEMS_MIDDLE and MENUITEMS_AFTER can be used to add links to the main navigation in this specific locations.

since this PR changes README.rst it will conflict with PR #60. so once you merge #60 i will update this one here.

hi duilio. updated this PR now. the second commit makes DISPLAY_FEEDS_ONMENU behave as all other DISPLAY*_ON_MENU (default is True).
