I tested this plugin with QGIS 2.18 and:

I can't configure the widget, the configuration ui is not displayed
In the attribute form, the widget displayed Failed to create widget with type 'formawarevaluerelationwidget'

The plugin was not well init. I have to restart QGIS. Restart the project wil probably be enough.
