8 years ago I've seen this as well but (a) It only happens with the Simple theme (for me), (b) Happens only to Fight-O-Matic and sometimes Track-O-Matic and (c) Happens randomly.
I can't consistently reproduce at present (for some reason) but it's a very strange bug as it's essentially just using built in wow capability. If you've got a way of consistenly causing it to happen I'd love to know (so I can track down the error)
Maybe it's because those things are LoadOnDemand.... hmm...
if you see where my pointer is on th eleft side of this screen shot - below vehicile controls and above addons is a blank space which is supposed to say svui or supervillain ui or w/e. for some reason its not showing up.
