Index - First Pass #68

Update with Chapter 4 edits.

I've completed a first pass at the index. If pull request is approved, it would be great if book-indexing could be recompiled. My next step is to look over the generated index and look for redundant entries/formatting issues/etc.

This is looking great!

I'm going through the index, and seeing a few things that could be fixed to make the entries consistent, e.g., both singular and plural for the same term, some compound terms with a hyphen, some without, etc. The main thing is that there shouldn't be variant versions of the same term as separate entries.

I'll send you a PDF scan or put it in the repo.

OK. When do you expect to finish? I just had an email from Michele saying she wants to send it to the printer by the end of the week. That means I'd love to have your part done by tomorrow.
