fix: remove unused MigrationMap model #19

Fixes #8.

As discussed in the original issue, MigrationMap model removed completely due to the being useless for most users. I have removed MigrationMap definition files, it's usage in other files, and changed README.md docs to match the new situation.

Please let me know if any other changes required.
