Task 2 - Photometry Form #34

Currently, in SkyPortal, the page (accessible through the source page itself) dedicated to adding new photometry allows you to copy and paste the content of a csv file and submit it.

However, it would be much better to ALSO have a more classic and user-friendly form where users can add the same data as the one they would add in the csv text field.

We have this kind of form in mind (for the format and style, then you need to add the right fields):

This task is a priority. Please focus on this one! We would love to add it to SkyPortal soon enough so that it can be used during the current GRB/ZTF campaign. Do not open new pull requests (unless the code is ready of course) other than this one.
