Add new fields to LabHead

Add the following fields (in order) to LabHead, between the Lab Name and Email fields:

• HMS Quad Appointment
• HMS Appointing Department
• Date Appointment Last Updated

The 3 categories for HMS Quad Appointment are:

• blank (nothing in excel document for this person)
• HMS Quad Faculty
• HMS Affiliate with HMS Quad Appointment
