Samples sent to Illumina - no longer exist.

Also, when I forked this file, it converted the file to "UTF-8". Notice in the comparison (i.e. split view) that there are "^M" at the end of each line. This is a Microsoft return character. I'm guessing this CSV was exported from Excel. Forking this file eliminated all of these and that's why it appears there are so many changes...
