[ ] Mongoose Schema for Treatment Detail Exception
[ ] Repo module for treatment detail exceptions with the following operations:
[ ] createOrUpdate (replace the existing exception)
[ ] get by program ID
[ ] list all - returns a summary of each exception, replacing the array of donor IDs with the count of donors in the exception
