Add sample_registration file to download templates from dictionary

• when I download the File Templates from the dictionary, it should include the sample_registration.tsv file.
• When I download v.12, I only got 13 files in the ZIP instead of the 14 listed on the dictionary site.

• the url being used in the dictionary sample_registration template as its currently used for the all option in the submission page which doesn't include sample_registration.
• we could add a parameter to return sample registration template too from clinical

• [ ] Update https://clinical.qa.argo.cancercollaboratory.org/api-docs/#/Dictionary/get_dictionary_template_all with a new parameter exclude_registration , which by default is set to false

this is working in production now - the docs template download includes the sample_registration file
