add pages of onboarding

now we have to move to POST and GET methods and form validation in onboarding_registration.html if you agree

@dezhizhang1985 I wonder that I should separate onboarding demo app from trust-anchor-intermediary app. Because someone must be able to use the trust-anchor-intermediary feature without take also the onboarding demo.

Tomorrow I'll do a refactor, after I'll merge this
