Describe how most unique deaths are determined #21

I never came round to write up what the conditions are for most unique deaths.

Also, how the deaths were normalized was really ad-hoc and not really well thought out.

For example, I'm not sure if "hallucinogen-distorted" really needs to be stripped.

FYI #48 pull request implements some of the widely-agreed-upon death normalisations. #43, #33, #21 seem to all be related to this.
