missing MARC mappings #86

A few rows in the table of header elements are missing values for the last two columns (the MARC mappings). I need to review the table with Adam Baron. It would be best to wait till https://github.com/kshawkin/Best-Practices-for-TEI-in-Libraries/issues/83 is done, but we may decide to leave that for a future revision to the BPTL while waiting on resolution from Council.

I've emailed Adam, explaining what's forthcoming in #83 (so as not to wait on that issue to be closed).
