Remove EBNF cross-references to save space #408

It takes ~11 pages of PDF and gives almost nothing. Might be good as an app with hyperlinks but pretty much useless for a printed book.

I use that cross-reference. It has different uses than an interactive search for the entries. la bremenli

The references all go to basically the same page

In the printed CLL, the EBNF cross-reference lists all the grammar production rules that mention a term on the RHS.That is very useful.If you are removing some other kind of page index for terms in the EBNF, that's fine. Vincent Bromanla bremenli
