Indices have lots of almost duplicates for the same item #419

Example: .a: cmavo, sumti connection, Truth functions and corresponding logical connectives, Truth functions and corresponding logical connectives, Truth functions and corresponding logical connectives, Truth functions and corresponding logical connectives where Truth functions and corresponding logical connectives link to different parts of still the same section.

Expected: references to nearby paragraphs should be merged into one.

Should be merged only for HTML versions of the Book.
