Add ODM design specs #20

I think the best resource for understanding the meaning of each column is the ODM design spec. https://his.cuahsi.org/documents/odm1.1designspecifications.pdf

Can we add this to the repo, perhaps in the CUAHSI docs folder?

Add instructions on when the reader would want to refer to the specs, which is basically when they can't figure out a column.
