How it works:

Tables/entities are Form and FormItems
FormItem has kind, usually QUESTION but can be anything else (like heading, etc.) just like Google Form
FormItem.answerKind can be SHORT_TEXT (jawaban singkat) or SCALE (angka dari 1 sampai 5).

Defined forms can then be triggered, to send the form to a user (via Twitter) and collect feedback. (note to Hendy: in future this could work with jBPM or Drools rule)
