Labels and descriptions for full names and surnames are mixed up #109

In the registration form for Cooperators, the 'label' and the 'description' are mixed up between 'surname(s)' and 'name(s)'.

The description for surname(s) should contain the text relative to family names. The description for name(s) should contain the text relative to given names.
