Allow Multiple Home facilities for Doctor user type

Currently "homefacility" is limited to a single facility for the doctor user type. This creates difficulties/confusion among doctors who work in multiple facilities as treating doctor.

Describe the solution you'd like

• [ ] allow doctor user type to select multiple homefacilties on user creation form
• [ ] allow doctor user type to Link multiple homefacilties on Link Facility pop up
• [ ] Doctor's name should be coming up on treating doctor dropdown on consultation form of other home facilities.

Also what are the previlags of a doctor how has a home facility marked 🤔 Where do I get documentation for this feature?

@bodhish Home facility linked doctor (doctor on site)- Have full access to view and edit patient file, along with discussion notes.

Doctor without home facility linked (Remote/TeleICU Doctor)- Only view access to patient file. Can ONLY add/edit Discussion notes
