Refactors provider and models
Ensured all tests passed after refactoring as described in #88 here

@chinedu117 this branch still includes models for djangodemo and surface in the /opencdms/models/ directory. Could you move both into /provider/?

Also there is a merge conflict because the /test.db binary file that is created by tests/unit/test_schema.py is stored in the repo. Could you remove /test.db and also add this file to our .gitignore so that it's not added to the repo in future?
