Indexterms inside programlisting passthroughs don't get stripped out with strip-indexterms param #39

Programlistings with child elements are output in a passthrough using an xsl:copy-of. This logic may need to be reworked in order to suppress indexterms from being output with the rest of the programlisting when strip-indexterms is set to "true".
