NI often needs to follow up with end users; usually they use the method by which NI was contacted. Since this application circumvents that, the client would like the user to select how they should be contacted if follow-up is needed. The person filling out the application and the person the application is for are often different, so this should be editable. While we are guaranteed to have an email for each account, for many of the end users they rarely check it.
