Start Upload button grayed out #10

Hi there. On my site here, http://www.cedarparkprinting.com/upload/ after I upload a pdf (which I've allowed as an extension), the Start Upload button is inactive.

I've disabled all other plugins too to see if that was the issue and still it is grayed out. I've tried Safari on a Mac and FF in a PC.

Any ideas?
