Create template needs to take in the following fields and assign them to a new template

This should be implemented at /Templates/Create and take the following input to create a new template

• List of Criteria
• List of Fields
• List of uploaded forms
• Name
• Description

Don't worry about filters yet!

This function, at url /Templates/Create should take the following input on the screen and add them to a new template.

General code flow should be:

• Check the Template
• Save the new template
• call filehandler.SaveForm(bytes,name,template id,kind) for each uploaded file.
