Bring across body generation code #8

We have body generation code for CC-collected markup - this will need to be run as a separate step on CC JSON prior to import. It formats sections in an agreed order to an agreed style suitable for initial import.

Looking at this now, our old body generation code munged together OFT and CC content after merge, under h2s ## Phase 1 and ## Phase 2 respectively. At present, I'm just taking out the OFT tests and code and leaving one weird-looking Phase 2 header. @adammaddison, does that sound reasonable?
