Link the CC sheet to the crawler #9

As we create cases (or afterwards if necessary, given that our root original_url is actually a summary and not the same as the one in the spreadsheet), look up the extra values from the sheet that aren't on the site. Fill them in.

CMA::CaseStore::Index created in bf204bef97fb73607f46b2ee51d48657020fdc85 to support the lookup of cases by any URL.
