Make sure they work properly

Display correctly
good enough wording
record answers

(to see responses use database reader - in meteor dir:)

@nocturnedesign and @kevinWillis could you take the lead on the testing. Well sort any last minute changes out in the am/mid afternoon before our 7pm time. Should be fairly explanatory.

Video path does not seem to be the same as the file recorded. I'm investigation.
