Context

Kobo-Connect now gives internal server error if there is no "choices" sheet, eg if there is no select_one and select_multiple question

What needs to be done

[ ] Review code and update where necessary, so that there will be no error if there is no "choices" sheet in the XLS form

Acceptance Criteria

[ ] Kobo-Connect can handle XLS forms without "choices" sheet
