Add option to build SQL create table statement

Goals

• Calling cutplace --sql table some_icd.ods write a SQL create table statement
• The option --table TABLE allows to specify a table name; default is `'${table}', which can later be filled in by a template processor
• The API provides means to easily write code that build create table statements
