Gem updates to rails6 #84

Doing this as a single big-bang commit, because it's a relatively light load:

had to make use of the unreleased master branch of our teaspoon JS test driver for railties compatibility
run the update generator rails app:upgrade
then fix the suddenly failing spec (activesupport seems to handle incremental attachments somewhat differently; add them all at once addresses the issue)

You're right about the database changes -- there are a couple of extensions that I have loaded in my local template01 database that get enabled automatically.

I'm pretty sure that we actually want the citext extension enabled, and to have a couple of our name fields translated to that type. Need to think about the other one. Thanks for the head's up.
