Initially just an overall comments box. A separate feature will include structured comments for individual resources

@chrismclarke - This is what you suggested earlier today: a quick form saying "what's the bug / what happened", "what did you expect to happen", "how to recreate the bug" that gets sent to us at support@stats. (Or adds a new issue to the Github board? ... I guess email will be easier to setup...)
