Change "plug-in" to "plugin.
Change "MarkDown" to "Markdown".
Change "BBPress" to "bbPress".
Fix other typos, inconsistent whitespace and add full stops.
Move changes from readme.md into new CHANGELOG.md and format it per keepachangelog.com.
