Show thumbnail in hold list and checkout list #36

Currently, we are just showing the title and author text.

Title and author columns can be combined; the leftmost column can be used for 
the thumbnail. The thumbnail URL is supplied in the metadata object for each 
title. We would need to download the thumbnail after receiving the metadata 
object.

Done for hold list, checkout list, and also holdings list in place hold form. 
The image is placed in the column that was showing title. Title is now merged 
with Author column. Image has alt property set to title.

What should show if thumbnail is clicked?
