Replace Zendesk feedback code with Web Widget #141

Zendesk has phased out their current Feedback Tab as it currently appears on Stormpath.com.

We need to update the code snippet to:

Zendesk is ending support for the Feedback tab on Aug 1. Please update before then. Thanks!
