Move entity docs from README to schema.graphql file #25

As @r-czajkowski proposes in https://github.com/threshold-network/subgraph/pull/18#discussion_r917761766, it would be a good idea to move the documentation of subgraph entities ( GraphQL relevant entities section in https://raw.githubusercontent.com/threshold-network/subgraph/b3c8ad6284a0479b368252baaf41136e37f72d63/README.adoc) to sechema.graphql file as comments.
