Document connections to be stand alone for each cli command #63

As recently discussed, we will implement the connection procedure stand-alone for each vsw cli command. So each command to the repo is like this:

establish a connection between the user and the repo
do the command protocol
tear down the connection

Update the flow chart to reflect this.

This model may be OK for the individual users at this time but is not a good choice for the repo. See discussions on "Threading" in aca-py. We will revisit this after the simple case is working first.

Confirmed that this issue is incorrect. Connections between parties (DIDs) persist. Won't change and closed.
